Endometriosis and fertility decisions

A patient discussing healthcare with a clinician

Endometriosis and fertility decisions need to be considered together. The best approach depends on symptoms, age, ovarian reserve, previous operations and other fertility factors, not simply on the presence of the diagnosis.

Define the immediate priority

Tell the clinician whether you are trying to conceive now, planning for later or mainly seeking pain relief. Some treatments used to suppress symptoms are not designed to help conception while they are being taken. Ask how the proposed plan fits your current goal.

Assess the wider picture

Partner factors, tubal function and other findings may influence the choice between observation, surgery and assisted conception. Bring reports from previous treatment and operations. Ask whether fertility and endometriosis specialists should discuss the case together when the decisions overlap.

Discuss surgery carefully

Surgery may be useful in selected situations, but it is not an automatic fertility improvement for every patient. Procedures involving the ovary can affect ovarian tissue and reserve. Ask what benefit is expected, which risks are relevant and whether fertility preservation or IVF should be discussed before an operation.

Agree how the plan will be reviewed

Ask what would trigger a change in approach and how much time is reasonable before reassessment. Consider pain, treatment burden and emotional wellbeing alongside the chance of pregnancy. If different specialists give different recommendations, request the reasoning behind each option in writing. A good decision explains the trade-offs for your circumstances without promising that removing visible disease will eliminate every symptom or guarantee a baby.
