Painful breastfeeding: when to ask for help

Persistent breastfeeding pain deserves attention. The cause cannot always be identified from the timing of pain alone, and different problems require different approaches.

Begin with a feeding assessment

Positioning and attachment are common contributors to sore nipples. Ask a trained professional to observe a feed and examine any nipple damage where appropriate. Explain whether pain happens at attachment, throughout the feed or between feeds, and whether it affects one or both sides.

Describe associated symptoms

Mention redness, swelling, a tender area, fever or flu-like feelings. Tell the clinician about broken skin, previous treatment and any symptoms in the baby. Do not assume that every burning pain is thrush or start repeated treatments without reassessment. Skin conditions and other causes may need different management.

Ask for a feeding and comfort plan

Discuss how to keep the baby fed while the cause is addressed. If direct feeding is difficult, ask whether expressing is appropriate and how to do it comfortably. Check any medicine for suitability during breastfeeding with the treating clinician or pharmacist. Avoid applying multiple products or using forceful massage on a painful area without advice.

Review rather than endure

Ask when improvement should occur and what to do if it does not. Worsening redness, fever, significant swelling or feeling unwell needs prompt medical advice. If the baby is not feeding effectively or appears unwell, arrange paediatric assessment as well. A good plan addresses both comfort and milk transfer, with a clear follow-up point instead of an instruction to simply keep going through increasing pain.
